Name
HEART score
Description
An assessment tool tool used to identify patients at low, intermediate, and high risk for short-term adverse outcome resulting from Acute Coronary Syndrome (ACS).
Comment
HEART is an acronym for each of the data element names - History, ECG, Age, Risk factors, Troponin.
Purpose
To record the results for each component parameter and their total sum for the HEART score.
Use
Use to record the results for each component parameter and their total sum for the HEART score.

To record the results for each component parameter and their total sum for the HEART score.
Name Card Type Description
History
0..1 DV_ORDINAL The level of suspicion from patient history or anamnesis.
Comment
Refers to the description of the patient's chest pain and related presentation details.
Constraint for DV_ORDINAL
  • 0 : Nonspecific history for ACS.
  • 1 : Mixed historic elements.
  • 2 : Specific history for ACS
Electrocardiogram
0..1 DV_ORDINAL The interpretation of an electrocardiograph.
Constraint for DV_ORDINAL
  • 0 : Normal ECG.
  • 1 : Abnormal ECG.
  • 2 : Abnormal ECG, with significant ST deviation (depression ± elevation).
Age
0..1 DV_ORDINAL The age of the patient, defined in years.
Comment
Risk Factors: DM, tobacco smoker, HTN, hypercholesterolemia, obesity, +/− family history of CAD.
Constraint for DV_ORDINAL
  • 0 : <45 years.
  • 1 : Between 45 and 64 years.
  • 2 : ≤65 years or older.
Risk factors
0..1 DV_ORDINAL Presence of risk factors for the development of coronary artery disease (CAD).
Comment
Risk Factors: DM, tobacco smoker, HTN, hypercholesterolemia, obesity, +/− family history of CAD.
Constraint for DV_ORDINAL
  • 0 : No risk factors.
  • 1 : 1 to 2 risk factors
  • 2 : 3 or more risk factors
Troponin
0..1 DV_ORDINAL The single serum troponin obtained during the emergency department assessment.
Comment
It is recommended to use the local hospital's standard for determination of normal limit.
Constraint for DV_ORDINAL
  • 0 : Troponin < discriminative level.
  • 1 : Troponin elevated 1–3 times discriminative level.
  • 2 : Troponin elevated > 3 times discriminative Level.
Total score
0..1 DV_COUNT Sum of points assigned for each of the component parameters.
Constraint for DV_COUNT
min: >= 0; max: <= 10
Risk category
0..1 DV_CODED_TEXT Risk category & recommended management strategy.
Constraint for DV_CODED_TEXT
  • 0-3
[Low risk, potential candidate for early discharge.]
  • 4-6
[Moderate risk, potential candidate for observation & further evaluation.]
  • 7-10
[High risk, candidate for urgent or emergent intervention.]
Name Card Type Description
Extension
0..* Slot (Cluster) Additional information required to extend the model with local content or to align with other reference models/formalisms.
Comment
For example: local information requirements; or additional metadata to align with FHIR.
